California is the first state to establish a health insurance exchange in accordance with the federal health reform law, with Gov. Arnold Schwarzenegger (R) signing the bill on September 30. "Every state is going to have to have some sort of legislative action on this," says Kansas Insurance Commissioner Sandy Praeger, chair of the National Association of Insurance Commissioners' (NAIC) Health Insurance and Managed Care Committee, adding that state agencies or private nonprofits can run the exchanges. A five-member board, which will assemble in March 2011, will run the California exchange. The NAIC and the American Medical Association have partnered on a model health benefit exchange bill.
