PIA Provides ACORD Forms for Use In Disaster Areas

ACORD has provided PIA with 5 forms for use by PIA members and affiliates in areas of the country affected by the several recent natural catastrophes. PIA members in disaster impacted areas are among the first-responders in their devastated communities. Claims processing begins almost immediately and under varying conditions. To support the timely processing of insurance claims, ACORD has provided PIA with current ACORD claims forms for our convenience, for use without charge in the disaster-affected areas:

  1. Property Loss Notice
  2. Automobile Loss Notice
  3. General Liability Notice of Occurrence/Claim
  4. Workers Compensation - First Report of Injury or Illness
  5. Aircraft Loss Notice

We encourage all PIA affiliates to make these forms available to members. You can use the links to the forms posted on the PIA National website. If the member agency is still online and able to take and process claims online, they can download these forms.

If the PIA member agencies must set up their claims processing site without power, then hard copies of the forms can be printed off and brought to our members for their use. In this case, we suggest having double the number of forms required, along with lots of carbon paper. Place the carbon paper between two hard copies of the forms, complete the claim, keep the carbon copy for the agency files (later, the data can be inputted into the agency system), and bundle the originals to ship to the insurers' claims' processing centers.  We encourage originals be sent to the insurers as hard copy notice of the claim, even if the claims information has first been called in by the agency to the insurer.
